Passes & Forms

All official passes used at BHS will be issued by the Student Affairs Office to faculty and staff as needed. Please secure all passes. Great care should be exercised by all to insure that passes are completely and properly filled out. Exact time, destination, etc. must be noted.

  • Permit to Leave Class form (pink) Students who leave class for any reason must secure written permission of the teacher in charge or must have in possession a request from another staff member. Any student outside of his designated classroom must show the pass to any BHS authorized personnel upon request. All teachers should collect passes. (Originator – classroom teacher)

  • Detention Notice form (white/yellow) This form is to be issued to a student who is to have a detention. If a student has more than one detention assigned, preference will be given to the teacher issuing the first written notice – (Originator – teacher issuing detention)

  • Admit to Class form (white) This form is to be issued to students who have been detained by BHS office personnel. Students having this form are to be admitted without penalty to the student. No tardy blank is to be issued.  Teacher should sign the slip before returning it to the Attendance Center (Originator – any staff member)

  • Office Request form (green or orange) This form is to be sent to the teacher when a student is requested to appear in a certain place as designated. Do not honor these passes if the origin appears questionable (Originator – Counseling and Administrative Offices)

  • Permit to Visit form (white) All visitors must have the proper identification badge in their possession as issued by the hall monitors at the entrance doors before visiting anywhere at BHS. Teachers are not to allow anyone other than students properly enrolled and in possession of identification badges in their classes without proper authorization from one of the Assistant Principals.

  • Notice of Failure to Master Significant Course Requirement form (white) Students who have not attained mastery of a significant course outcome are to be notified in writing of the possibility of the forfeiture of credit. Students, parents, guidance and administrative personnel are to be notified of the failure to master a significant outcome. They should be advised of the specific significant outcome, the exact date of the deadline (typically two weeks from notification), what will be required to accomplish mastery, and the consequence for failure to complete mastery by the deadline.

  • Request for Sponsorship of Field Trip Packet - Available in the Student Affairs Office.

  • Maintenance Request form - Use this form for repairs, to have items moved, and minor improvements. This form is available from the mail/supply room in the main office and is returned to the Principal’s Office for signature.

  • Supply Request From – use this form to request supplies available from the warehouse. This form is available from the main office and should be turned in to the receptionist.
