Homebound Students

Requests by parents for homebound services are usually made to the school nurse or to the counselor.  The counselor completes a “Request for Homebound Teacher” form, and includes information relative to current performance level and latest achievement test data, if available.  This information may be photocopied and attached or indicted on the form itself.  The current and adjusted schedule should be provided by the counselor, including the names of the teachers.

The counselor routes the “Request” through the Guidance Office Assistant who makes a copy for herself, one for the Assistant Principal, and forwards the originals to the Director of Special Services.  The Director will assign the homebound teacher upon receipt of a medical certification from the physician or direct consultation with the physician and returns the white copy of the request to the counselor.

The Director of Special Services will inform the appropriate Assistant Principal when the teacher assignment has been made, send a copy of the medical certification to the school nurse, after which, the following procedures must take place:

 

  • The Assistant Principal will schedule a placement conference with the counselor, classroom teachers, and homebound teacher and encourage the parents and the student to attend the conference.  The Assistant Principal will indicate to the parents/student that a Notification of Educational Placement Plan will arrive through the mail if the parent does not attend.  Once approved, it is the responsibility of the regular classroom teacher to provide the homebound teacher with assignments, examinations, etc., upon request.  Any problems experienced by either the homebound teacher or regular classroom teacher in regard to assignments, etc., should be brought to the attention of the appropriate Assistant Principal immediately.

 

  • At the conference, an IEP Summary Sheet will be completed by the Assistant Principal including the educational goals for the homebound student.  Materials and assignments for the homebound teacher should be made available by the teacher.

 

  • Following the conference, the Assistant Principal will send a Notification of Educational Plan to be signed by the parent/guardian (or student, if married).  The Assistant Principal will maintain a copy of the Notification to the Director of Special Services.

 

  • Upon receipt of the Notification of Educational Plan, the Assistant Principal will forward the IEP Summary and the Notification to the Director of Special Services.

 

  • The homebound teacher may begin instruction prior to the time the signed Notification is returned.
