Health Examination and Immunization
Requirements:
All high school students in Illinois are
required to show proof of having met physical examination and immunization
requirements prescribed by the Illinois State Board of Education.
Documentation must be provided at registration when the student is first
enrolled at the high school.
Students previously enrolled in the Bloomington Schools will not be allowed
to register for school in the fall until proof of meeting physical
examination and immunization requirements is presented to school
authorities.
Students transferring to Bloomington High School from another school may
transfer their health records. If the records of a transfer student are not
received within 30 days of the request for such records, parents will be
informed and an additional 10 school days will be allowed to show proof of
meeting physical examination and immunization requirements. If proof of
meeting such requirements is not provided within the above time frames, the
transfer students will be excluded from school until proof is presented to
school authorities.
Health examination forms may be obtained from the school nurse or local
physicians. Students whose parents or legal guardians object to health
examination or immunizations on religious grounds shall present to school
authorities a signed statement of such objection on a form provided by the
schools. These forms are available in the nurse's office.
Dental examinations are encouraged by District 87 Schools, but are not
required by State Law.
Students or parents who have questions about physical examination and
immunization requirements are encouraged to contact the school nurse. Copies
of Board of Education Policy 7.100, Student Physical Examination and
Immunization Requirements, are available in the school office.
Vision and hearing screening will be done, at intervals mandated by the
State of Illinois, for all students in special education, students referred
by an educator and new and transfer students. Vision and/or hearing
screening are not optional. Unless a vision or hearing examination report
from an optometrist, ophthalmologist, or audiologist is on file at school
indicating that an examination has been administered within the previous 12
months for your child, your child will be screened. Vision and hearing
screening is an ongoing process throughout the school year.
School Nurse:
Students who need the services of the school
nurse must secure from the classroom teacher a Permit to Leave Class form.
Time of leaving the classroom is to be stated.
A student desiring to see the nurse at the close of a class must go to his
next class and remain until the classroom teacher issues the necessary form
to see the nurse. No student will be admitted to the Nurse's Office between
classes except for emergencies.
Students must present parent/guardian permission for a pre arranged doctor
or dentist appointment to the Attendance Office prior to leaving the
building. Students must sign out through the Attendance Office prior to
leaving the building for an appointment.
Emergency phone numbers will be maintained for all students and should be
filled out the day of registration. It is important to have the name and
phone number of a person other than the parent or guardian to call in case
of an emergency when parents cannot be reached.
Administering Student Medicine
It is policy of the Bloomington Public Schools
that as a regular and normal practice, medicine should not be administered
to a student at school or when the student is involved in school activities.
However, under certain circumstances, medicine may be administered during
school hours by the school nurse or self administered by a BHS student
provided the following rules are followed. These rules apply to both
prescription and over the counter medication.
A request for administering student medicine must be submitted in advance on
a form provided by the School District. This form is available in the
nurse's office, in most local doctors' offices, and is included at the end
of this handbook. The completed form is returned to the nurse with the
prescribed medication and the form is then approved by the principal. When a
physician prescribes that medicine should be given to or self administered
by the student at school, parents are advised to have the doctor complete
the request form immediately.

policy are available in the school office.
