 REQUIREMENTS FOR SUBSTITUTE TEACHERS

 

  1. A valid Illinois teaching certificate registered at the McLean/Dewitt/Livingston Regional office (905 N. Main Street, Normal 309-888-5120), we will copy for our files.  Or you may get a substitute teaching certificate issued/obtained at the Regional Office of Education.
  2. Substitute teaching application
  3. Resume’
  4. Copies of unofficial transcripts
  5. Tax forms (federal and state).  Please list your permanent address on both of these forms.  Your social security card will be copied for Payroll.
  6. TB test (administered within the last 90 days).  To obtain a free TB test, contact the McLean County Health Department, 309-888-5450.  They are located at 200 West Front Street, Bloomington.  It is necessary to schedule an appointment for this test.
  7. Fingerprint Identification form.  Please take this form to the Mclean County Jail department (located on Front Street) to be completed.  Please attach your receipt to the fingerprint form and return it with your completed paperwork.  District 87 will reimburse the five-dollar payment after you have been employed with the district.

Notice

 Effective immediately, pursuant to Public Act 093-0909 and 20 ILCS 2605/2605-325, certified and non-certified applicants for school employment must undergo a fingerprint based criminal history record check as a condition of employment.  The use of name inquiries is no longer permitted.   A fingerprint-based record check must be conducted through the Illinois State Police and Federal Bureau of Investigation criminal history record file for all certified and non-certified school applicants, with the exception of school bus drivers, according to the law.   Consequently,  name-based criminal history record checks will no longer be processed by the Illinois State Police Bureau of Identification for school employment purposes.

 

  1. Child Abuse Mandated Reporter form
  2. I-9 form (please bring your driver’s license and social security card with you for verification).  Your social security card will be copied for the Payroll department.
  3. Teachers Retirement System form
  4. View blood borne pathogen video or read the book and complete the 20-question true/false quiz within the back of the book.  The book is yours to keep.
