Kindergarten Requirements:

• Health history (completed by parent)
• Physical examination completed within 12 months prior to the first day of school, current immunizations (see list below).  Must be turned in to school ON or BEFORE the FIRST DAY OF SCHOOL.  Failure to comply will result in your child being excluded from school.  See the District 87 handbook for details http://www.district87.org/esc/boardpolicy/default.htm
• Immunization Requirements
• Vision Exam must be completed by OCTOBER 15TH in order to be in compliance with the state of IL requirements.  The exam must be completed by a certified optometrist/ophthalmologist.
• Dental Exam must be completed within the preceding 18 months of the May 15th due date for that school year. 
• Any student having special dietary needs must fill out the Physician Food Substitution Form.  We must have a copy on file with the nurse and cafeteria staff.

Second Grade:

• Dental Exam must be completed within the preceding 18 months of May 15th due date for that school year.

Out of State Transfers:

• You have 30 days from the day your child starts school to obtain an IL physical, current immunization records and vision exam.

If your child requires medication at school, prescribed or over the counter, a medication authorization form must be completed by the physician and the parent/guardian.  This includes medications such as Tylenol, Motrin, Benadryl, Tums, cough drops, Chapstick, etc.
