• Step 1) New Student Online Enrollment

    Step 2) Provide Documentation

    You can either upload the documents when completing the online application, email them to hospelhornj@district87.org, or bring them into the school to be copied. Pictures MUST be of the entire page (not just part of the document) and cannot be blurry. 

    Proof of Residency (We need 2 different documents from the following)

    • Property Tax Receipt
    • Utility Bill
    • Medical Insurance Card
    • Bank or credit card statement
    • Insurance policy or bill
    • Lease Agreement or Mortgage Statement (front page and signature page)

    Birth Certificate

    9th Grade Physical

    Current Immunization Records

    Temporary or Court Ordered Custody Documents (if you are not the mother or father enrolling the student)

    IEP or 504 documentation (if you have one)

     

    Even though I do request documentation from the school the student is transferring from, the enrollment process will be faster if you provide it before they get back to me.
